Vaccines Offered
- Influenza – Regular 5+ and High dose for those 65 and over.
- Pneumonia (Capvaxive) – talk with pharmacy staff to see if you are eligible for this new form.
- COVID-19 Moderna Spikevax(R) – Recommended for 12 years and older.
- Shingles (Shingrix) – Recommended for those over 50 years old, available as a two-dose series (2 to 6 months apart).
- TDAP – for those 12 years and older and greater than 10 years since the last dose of vaccine.
- RSV – Recommended for those 75 years and older, and for patients 60-74 years with qualifying medical conditions.

Please complete the Immunization Consent Form here, or if you have internet limitations contact the pharmacy for more options. This form needs to be completed at least 2 days before your scheduled appointment. Please wear comfortable clothing with easy access to your upper arm.
Please be ready to provide your current insurance card/Medicare card. Many insurance companies will cover the cost of vaccinations. We will also have cash pricing available.
